ACHC’s Patient Portal is a secure website that allows patients to view their personal health records, including labs, immunization records, and visit summaries. You can also view messages from staff, billing statements and demographic information. Our
Patient Portal helps us communicate with you by providing convenient 24/7 access from the comfort and privacy of your own home or office.
To set up your patient portal you simply need to contact one of our Patient Advocates and provide an email address. You will receive an email with a link to the Patient Portal and a secure username and password you can use to login and set up your account. You can login online or via the
Healow app.
